Hey @BigBoss, welcome to the forums.
I think they have opened up the playing of the game so most of what you do gives you gear. Getting to gs#256 shouldn't be hard.
Generally, I think the missions are effective way, esp. the daily. HVT is good too.

Agreed, missions and LZ boss runs would work well to beef up gear levels. Also, if you run w/ some 256 players they may rain loot on you if they get things they don't need.

I found Underground one of the best places for loot to level up... you just have to remember that each time you're about to open a Cache ...make sure you're wearing all your highest level gear. That way the drop from it is calculated on your current gear score average which means you get the highest gear from the cache.
