keep getting stuck in cover glitches

Happened 3 times today, character gets 'stuck' in cover, weapon dissapears and i cant move, Enemies seem to think ive dissappeared and cant find me even when looking at me, all i can do is reset my xbox.

Any ideas why ?

Does this happen all over the map or in a specific place?

It happened to me last night during a daily.  Fortunately the guys in my squad had it happen before.  They finished off the enemies in the area then backtracked to a point they were considered "Out of combat" and I was able to fast travel to our group leader.

Our group leader said it seems to glitch when two people duck into the same cover at the same time.

I had this happen to me twice in the Russian Embassy. In two different locations and fast travel was the only way out of it. Both times, I was the only person utilizing the cover location so that was not the issue in my case. Either way it is a minor bug.
