Used to be in development 20 years of experience in that profession the 2 most damaging processees that I encountered were QA and Repositories. Your QA sometimes only test the specific change and not the overall impact that it could have on other areas. In Reference to repository for example if a developer picks an outadated code and installs his changes without knowing that there is a newer version that causes tons of issues. After my research Division 2 has 3 layes the c++, snow drop it is the scripitng that ties all 3 , and finally the databse which i beleive is NoSQL. So there is a greater chance of errors if any of these layers fail. So it takes a lot of coordination between all layers and the question is do they have a QA for each layer or one overall QA that test all of the finalized changes.
